By submitting a request for cheerleaders, you acknowledge your agreement with and acceptance of these policies. Fill out all required fields; required fields are in bold type. Once your request has been reviewed, we will contact you to confirm your information and request any missing information. Then, in accordance with the Cheerleading Appearance Policies, we will let you know as soon as possible whether or not the cheerleaders are available for your event. If your request is accepted, it will be in accordance with the information you provide in this Cheerleading Appearance Request Form. We will try our best to accommodate changes requested after your initial request; however, we cannot guarantee that we will be able to fulfill them.
